PERSONAL INFORMATION PROTECTION AND ELECTRONIC DOCUMENT ACT

MJL/SCS employee accountable for Personal Information:

The purpose that MJL/SCS Disaster Kleenup collects Personal Information is to facilitate the claim handling procedure in order to:

  1. Assist the Client with evaluation and settlement of the claim.

b) Assist the Client in order to detect and prevent fraud.
c) Facilitate the completion of the restoration work.

The description of the type of personal information held by MJL/SCS Disaster Kleenup is as follows:  
Names, addresses and phone numbers of policy holders for who we have asked to do property loss appraisals.  This information may also include their place of employment, secondary residence with phone numbers and cell phone numbers.  The information may also include the name, address and phone number of a contact person.

Procedures to protect personal information:

  1. All personal information relating to claims shall be recorded on the companies standard forms as per claim handling procedures set out in the MJL/SCS Company manual. 
  2. Personal information will not be left unattended and left accessible to others.
  3. All personal information retained to facilitate claim procedures or as part of contents for the purpose of cleaning and storage, will be protected and secured by MJL SCS Building Access and Security Policy and Procedure.
  4. Project Managers may request specific security procedures as may be required pending the sensitivity of such information or contents, using the form provided.

Procedure for requesting Personal Information:
            Personal Information may only be obtained through the following procedure:

  1. By contacting the MJL/SCS Employee accountable for Personal Information.
  2. By providing confirmation that the information will be used only for the purposes for which it was originally obtained.
  3. Pre-approved of subcontractors and suppliers, provided the information is requested or passed on for the purpose as noted above.

Procedures that will respond to inquiries and complaints.

  1. Inquiries and complaints must be addressed to the MJL/SCS Employee that is accountable for Personal Information.
  2. Individuals with inquiries or complaints must provide confirmation that they are inquiring about their own personal information and state the purpose of the call.
  3. Company policy will be corrected as may be required and the individual will be informed of those corrections.
